Before your Back Story interview you will need to complete a detailed questionnaire and select your 10 music tracks.

The questionnaire can be completed in 3 ways.

Download and print to complete the questionnaire offline and return by post to Damian Bailey, 4 Lancaster Avenue, Farnham, Surrey GU9 8JY or by email to hello@back-story.co.uk.
By post. Please get in touch to let us know your address and we will send you a paper copy of the questionnaire.
